During World War II, the USS Tiger Shark, a submarine of the US Navy, patrols the Atlantic Ocean in August 1943. The submarine receives orders to pick up survivors spotted adrift by a British PBY Catalina patrol plane and rescues three survivors – British nurse Claire Paige (Olivia Williams) and two men, one of whom is wounded – from the sunk British hospital ship Fort James two days earlier; one survivor blames a German U-boat he spotted on the surface just before Fort James was torpedoed.
The crew of the Tiger Shark notices a German warship coming. The submarine is damaged by depth charges after repeated engagements with the destroyer. Lieutenant Brice (Bruce Greenwood), the commanding officer, realizes that the wounded survivor is actually a German POW, Bernhard Schillings (Jonathan Hartman). Brice confronts Schillings, believing he has been making noise to reveal Tiger Shark’s location to the German cruiser and shoots him dead when the German panics and grabs a scalpel to protect himself.
The crew of Tiger Shark loses control of the submarine due to a series of strange mechanical malfunctions, and the submarine returns to the site of the sinking of the German ship on her own initiative. Accidents are killing crew members at an alarming pace, and the crew suspects a supernatural factor, casting doubt on Brice’s explanation of Winters’ death.
Paige and Odell uncover that Tiger Shark sank the British ship, Fort James, after mistaking it for a German submarine tender; they also learn that Brice, Loomis, and Coors colluded to hide the tale, killing Winters on the deck as he tried to save the survivors of Fort James.
Accidents have debilitated Tiger Shark, leaving only five survivors: Brice, Odell, Paige, Stumbo (Jason Flemyng), and “Weird” Wally (Zach Galifianakis). Wally comes to the conclusion that the submarine is plagued by a “malediction” that must be fulfilled in order for it to leave its netherworld between heaven and hell. When the Tiger Shark gets to the site of the Fort James sinking and surfaces crippled, the crew notices a surface ship approaching. Paige sneaks out on the deck and tries to signal the ship with a flashlight, but Brice forbids the surviving crew from radioing the ship. Brice confronts her and threatens her with a revolver.
His guilt over the disaster overtakes him; he reveals the cover-up and shoots himself in the head, dying in the water.
The four survivors are picked up by the ship Paige, which turns out to be British. The Tiger Shark descends to the ocean below, close to the ruins of Fort James.
